The B-2 visa for medical purposes has specific documents peculiar to it that are a must-have when applying for it. These are some of the most important documents that would be requested for:

  • A Form DS-160 (Nonimmigrant Visa Application). This is to be filled online on the DS-160 page and printed out.
  • An original passport following the appropriate validity and issuance period (6 months and 10 years respectively). The passport should have at least 2 blank pages for the visa stamp.
  • A passport-sized photo following the right visa photo requirement for the USA.
  • Your visa application fee receipt.
  • A cover letter from the patient's local physician showing the patient needs care in the U.S.
  • A record of the patient's illness signed by the local physician. This record should contain all the necessary details on previous treatments and tests that have been carried out on the patient. There should also be records of the patient's reactions to certain treatments.
  • Bank statements and other records of showing the amount you have for medical fees and other expenses such as housing, feeding, and transportation.
  • A Form I-134 (Affidavit of Support): Those who do not have enough can give details of a relative or friend in the U.S. who has agreed to sponsor the applicant. This would be done through a Form I-134 and the details should include the name, employment, income, bank accounts, and personal property of the host/sponsor. The form is a contract that binds the sponsor and the U.S government.
  • A letter from the healthcare professional you seek to visit in the USA showing that they approve of your visit to the USA for treatment. This letter must contain the cost of the medical services and a detailed treatment plan for your medical condition.

Applicants of this visa are advised not to falsify any information on their documents. You are also to ensure that you have complete documents. If you would be accompanied by a relative or a friend, they are to apply for their own visas separately from yours but indicate that they would be accompanying you.
