A letter from the receiving medical institution in Portugal stating willingness to treat the applicant is required.
Any other correspondence between the medical institutions in Nigeria and Portugal should be provided.
Applicants of this visa are required to provide proof that they can pay the medical bill and any other cost that may arise.
If the applicant of this visa is a minor, the unabridged copy of the minor’s birth certificate is required.
Non-Nigerians applying for this visa should ensure they are in possession of their Nigerian residence permit.
You are expected to leave the country on or before this visa expires.
