Medical Visa from Qatar to Belgium
- The visa application form must be duly completed and signed.
- One recent passport size photograph
- Original passport with a validity of three months after date of exit from Belgium.
- The passport must have at least 2 blank pages for Belgium visa stamp.
- Qatar residence permit is required from non-nationals of Qatar.
- Medical letter from Belgium stating the duration and purpose of treatment along with the applicant
details.
- Medical attestation stating the applicant is need of medical treatment and it is nonmedical
treatment available in Qatar.
- Flight ticket reservation is required.
- Hotel Reservation/Letter with precise name and address, from the person who is going to accommodate
applicant (this person may be different from the sponsor.
- A copy of a valid travel insurance certificate is required.
