Medical Visa from Chad to Colombia
- Applicants of the Colombian medical visa seek medical treatment in Colombia.
- You are to provide a letter from the health facility in Colombia confirming that you are to be admitted for treatment and the duration of the treatment is to be sent to the embassy.
- Copy of payment receipt of fees for medical treatment.
- A local report from your doctor stating your medical condition and the reason the treatment will be best gotten in Colombia should also be submitted.
- All the documents should be written in English or Spanish on the institution's letterhead and signed by the person in charge.
- If you are employed, a letter from your employer stating the allowance of your leave is required. Your status in the company and the duration of your employment should also be stated in the letter.
- Applicants are required to submit a document which shows that they have the incentive of returning to their country upon the completion of the treatment.
- Minors are expected to submit a copy of their birth certificate and a letter of consent from the parents if they are travelling with one or none of their parents.
- Proof that you have the financial means to cover the medical bill is to be submitted by the applicant.
- Proof of payment of visa application fee should be presented during the application process.
- Proof of their confirm appointment date from the Colombia hospital along with your medical situation.
