Medical Visa from Zambia to Italy
- This visa is for applicants that are traveling to Italy for the purpose of seeking and receiving medical treatment.
- This visa is also for those who wish to accompany sick applicants to Italy for their treatments.
- A local medical report issued by a reputable doctor /clinic/hospital in your home country, confirming your health status is required.
- This letter must confirm that your treatment can not be gotten in Zambia.
- A medical attestation is also required from the hospital or doctor in Italy, confirming the date of your appointment as well as your medical situation.
- It should also confirm that the hospital can perform the specific medical treatment and that you will be accepted accordingly.
- Proof that the applicant is able to pay for the medical fees must be submitted.
- If the applicant is unable to fund the bills, a sponsorship letter is to be provided.
- Bank statements of the sponsor are to be also provided.
- Official confirmation of the treatment/appointment by the receiving medical institution(hospital/clinic)in Italy is important.
