Medical Visa from Rwanda to Slovakia
- This visa is
issued to foreign citizens to allow them to enter the Republic of Slovakia to
receive medical treatment for their health condition.
- A correctly
filled and signed visa application form must be submitted.
- Two identical
passport-size photographs no older than three months are required.
- The passport used for your application must be original with at least three months validity beyond your intended date of departure from
Slovakia.
- Travel medical
insurance with minimal coverage of €30,000 to cover the duration of your stay
in Slovakia.
- You should also provide proof of
accommodation for the duration of your stay. This could be in the form of a
confirmed accommodation in the hospital or medical clinic you are visiting, a
hotel booking, a letter of invitation from a resident in Slovakia or a rental
contract.
- Proof of your civil
status is also required. This may be a marriage certificate (if married), birth certificate of
children (if any), death certificate of spouse, if applicable.
- To show that you can pay
the medical expenses and other related expenses throughout your treatment, you are required to provide proof. This could be your bank statements; letter of sponsorship with bank
statements of the sponsor; proof of advance payment for the treatment plus bank
statement for other related expenses and; if the ministry of health in Rwanda
will be covering the cost of your health treatment, a letter declaring the readiness of the government to cover your medical treatment costs.
- If you are employed, you
should provide proof of employment in the form of an employment contract,
approved letter of leave from your company, payslips for the last six months
and an Income Tax Return.
- If you are self-employed,
you should provide a copy of your business license, your company's bank
statement for the latest six months and an Income Tax Return.
- If you are a
student, you should provide proof of university enrollment and a No Objection
Letter from your university.
- If an underage is
travelling for treatment, both the original birth certificate with a copy;
letter of consent from both parents (if travelling with a third person) or from
the other parent (if travelling with one parent), or; death certificate of
parent (if one or both parents have passed away), must be provided.
- You need to provide proof of your need for health treatment. The following are needed:
- A letter from a doctor in Rwanda that you need medical treatment in Slovakia.
- Official confirmation of treatment by the receiving hospital or clinic in Slovakia.
- Correspondence between the sending medical doctor and the receiving medical doctor.
- A guarantee letter signed by you as proof that you can cope with the extra medical costs in case of unforeseen costs.
